LUMA Sportz N Fun | Level Up Initiatives LLC – Parent Contract 2026–2027

I, __________________, commit myself to and acknowledge the following:

LUMA Sportz N Fun (including Little Warriors, Little Pom Poms, Little Starlets, and Little All Stars) is committed to making this an exceptional experience for my child and myself.

• I acknowledge the inherent risk of injury by enrolling my child in LUMA Sportz. A signed liability waiver and parent contract are required.

• I understand that LUMA Sportz N Fun is not a licensed caregiver through the State of Texas and offers single-skill instruction in various sports.

• All communications, including event notices, class schedules, and updates, are sent via email and text. I agree to keep my contact information up to date.

• Tuition is due on the 1st of each month and remains the same each month. Payments are automatically drafted.

• There are no pauses in payment due to vacations, travel, or other absences.

• A 30-day written notice is required to cancel a membership. Without notice, billing will continue.

• All mandatory events are billed to each account in advance. These fees are non-refundable.

Membership Options

$70 Monthly Contract

• Cancellations require approval by both parties.
• A $70 cancellation fee applies if terminated before August 22, 2027.
• Virtual classes in August are included in the contract.

Early Termination
• If terminated early by the client, the client is responsible for all incurred fees and a $70 cancellation fee.

Non-Payment of Cancellation Fee
• If the client disables payment or disputes the charge and wins, LUMA Sportz N Fun reserves the right to manually bill the remaining contract through August 22, 2027.

$80 Month-to-Month
• Cancel anytime.

• Virtual classes in August are included if not canceled. 

Multiple Children & Program Discounts

Each LUMA Sportz N Fun program requires a separate membership, as every class features its own sport-specific curriculum and custom lesson plan.

Multiple Children: Enroll more than one child and receive 10% off the second child’s membership.

All 3 Programs: Enroll in all three LUMA programs and receive special bundled pricing:

Yearly Membership: $60/month per program — $180/month total

Month-to-Month: $70/month per program — $210/month total

Discounts are applied to qualifying memberships. Each program enrollment remains a separate membership.

Membership Change Days

We understand that families’ needs can change throughout the year! To provide flexibility, LUMA Sportz N Fun will offer two Membership Change Days each year—one in the Fall and one in the Spring.

During these designated days, families will have the opportunity to switch their membership from a Yearly Contract to Month-to-Month, or from Month-to-Month to a Yearly Contract. We will notify families in advance when each Membership Change Day is approaching.

Please Choose Your Membership Carefully: If you are unsure about committing to the program for the full year through August 2027, we strongly recommend selecting our Month-to-Month Membership. The Yearly Contract is designed for families who are ready to make a full-year commitment to the program.

Our goal is to provide families with options while keeping our membership policies clear and consistent for everyone.

Important Membership & Summer Billing Information

Our memberships do not automatically end when the school year ends in May.

Yearly Memberships: Your membership is a full-year agreement and remains active through the end of August 2027.

Month-to-Month Memberships: Our $80/month memberships also remain active through the summer unless a cancellation request is submitted according to our cancellation policy.

June & July: Tuition will only be paused for June and/or July if your child’s school does not host LUMA classes during those months. If classes are offered, regular tuition will continue.

Please remember that the end of the school year does not automatically cancel your LUMA Sportz N Fun membership. Families wishing to discontinue a Month-to-Month membership must submit their cancellation request before August according to our cancellation policy.

Summer Policy & Renewals

• No refunds during summer months. Billing continues unless you notify us of withdrawal.

• Memberships automatically renew on August 23, 2027 unless canceled.

• Schools that do not hold classes during June and July will have their associated student accounts automatically frozen for those months. All accounts will be reactivated for virtual classes beginning in August 2027.

• Frozen Accounts Policy: If payment is canceled or removed to avoid the August tuition, a $70 cancellation fee will be manually applied to the account for Monthly Contract holders.

Zero Tolerance Policy (Little Warriors Only)

LUMA Sportz N Fun maintains a zero-tolerance policy to protect students, staff, and partners. This policy applies only to Little Warriors Martial Arts participants.

• $35: Little Pom Pom / Little Starlet Showcase (Dec 6–10, 2026)

• $35: All Star Showcase (Dec 6–10, 2026)

• $60: Stick Combat Tournament (March 2027)

• $60: Spring Cheer Camp (March 2027)

• $60: All Star Showdown (March 2027)
• $35: Little Warrior Showcase (May 3–7, 2027)
• $35: Little Pom Pom / Little Starlet Showcase (May 3–7, 2027)

• $35: All Star Showcase (May 3–7, 2027)

Missed Classes

• We do not make up classes for bad weather, holidays, or unforeseen school closures.

• If LUMA cancels a class, parents will be notified, and the class will be rescheduled or credited.

• Missed classes by students will not be rescheduled or credited.

No Classes:
• Nov 23–27, 2026 (Thanksgiving)
• Dec 21– 31, 2026 (Christmas/New Year)
• May 24–28 & Jun 28–Jul 2, 2027 (Summer Break)

Dress Code by Program

Karate: Students can wear their school clothes to class. The Little Warrior T-shirt, karate belt, & Karate uniforms are optional & available for purchase on our proshop. Karate Students will receive a complimentary headband. 

Cheerleading / Dance: Students can wear their school clothes to class. The Little Pom Poms shirt, pom poms, & cheer outfit are optional for class & available on our proshop. Full outfit required for Spring Cheer Camp only.

Soccer & Basketball: Students can wear their school clothes to class. The Little All Star shirt, Jersey, or warm-up suit are optional for class & can be purchased in our proshop. 

Membership disputes, disputed charges policies

If any disputes or concerns arise during the year, LUMA Sportz N Fun will provide a signed copy of your Parent Contract, confirming your acknowledgment and agreement to the terms. This contract serves as a binding agreement outlining the structure of our program, what to expect, and our mutual responsibilities. All situations will be reviewed and addressed on a case-by-case basis.

If any charges from Level Up Initiatives LLC | LUMA Sportz N Fun are disputed with your banking institution and the outcome is decided in your favor, a manual invoice may still be issued to your account if supporting evidence provided by LUMA Sportz N Fun overrides the findings of the bank.

I acknowledge that I have read and understand the contents of this Parent Contract, including tuition, event fees, policies, schedules, and attire.

LUMA Sportz N Fun | Level Up Initiatives LLC - Combined Liability Waiver

Please read and sign each section below. All agreements must be completed and signed for students to participate in any of the LUMA Sportz N Fun programs, including Little Warriors Martial Arts, Little Pom Poms Cheerleading, Little Starlets Dance, and Little All Stars Basketball & Soccer.

General Liability Waiver

I hereby certify that my child (or I, myself) has my approval to participate in the LUMA Sportz N Fun programs offered by Level Up Initiatives LLC dba LUMA Sportz N Fun. I agree to assume all risks of personal injury that my child (or I, myself) may suffer while participating in LUMA Sportz N Fun and all related activities. I release and discharge LUMA Sportz N Fun, its officers, directors, employees, and volunteers from all liability, including claims, expenses, damages, or lawsuits arising directly or indirectly from participation.

In the event my child (or I, myself) is injured or becomes ill, I grant permission to LUMA Sportz N Fun staff to render or secure necessary medical treatment.

I grant permission for my child (or I, myself) to be photographed, recorded, and/or interviewed for promotional or evaluative purposes and understand that these materials may be used publicly. All evaluation participants are guaranteed confidentiality, and no individual student data will be shared. I release LUMA Sportz N Fun and its partners from liability relating to use of such materials.

I understand that LUMA Sportz N Fun is not a licensed caregiver through the State of Texas. The program provides single-skill sports instruction only.

Payment & Program Information Agreement

Monthly and full auto-draft payments will be processed through Square on the 1st of each month. The contract will be included in the first invoice. A 30-day written notice is required to cancel a membership. Early termination of monthly contracts before the agreed contract end date will result in a $70 cancellation fee. Invoices will continue unless proper notice is provided. Late payments over 7 days will incur a $25 late fee, and services may be suspended until payment is made.

Memberships will automatically renew on August 23, 2027. Please provide 30 days' written notice if you do not wish to renew. Tuition remains the same every month. No refunds are given for uniforms once received, regardless of withdrawal. Spring Cheer Camp and other mandatory events will be billed in advance.

Holidays and closings follow your local ISD district calendar. Coach or instructor absences will be communicated with make-up info sent via email.

Zero Tolerance Hitting Policy (Little Warriors Only)

The LUMA Sportz Little Warriors Martial Arts Program enforces a strict Zero Tolerance Hitting Policy to ensure the safety of all participants. Students are reminded each class not to use Martial Arts on others inappropriately. We address incidents on a case-by-case basis and maintain open communication with staff and parents.

If a student hits or strikes another student or teacher intentionally or through horseplay, they may be suspended from class that day. Continued behavior will result in a one-month suspension (no charge for the suspended month). Further infractions may result in permanent removal from the program.

Our goal is to teach respect, discipline, and self-control through martial arts. These skills are not to be misused against peers, family, or school staff.

Vendor Policy

As an approved vendor, LUMA Sportz N Fun and its instructors/coaches comply with all documentation and policies required by partner schools to operate during school hours and activities. This statement is subject to change based on experience, new information, and evolving resources provided to or by Level Up Initiatives LLC.

This combined liability waiver applies to all families enrolling in the LUMA Sportz N Fun programs, including Little Warriors Martial Arts, Little Pom Poms Cheerleading, Little Starlets Dance, and Little All Stars Basketball & Soccer.
