Please read and sign each box below. All agreements must be completed and signed for students to participate in the Level Up Martial Arts Little Warrior Program.
I hereby certify that my child (I, myself) has my (I, myself) approval to participate in the Level Up Martial Arts program, school (s) offered per above by Level Up Initiatives LLC dba Level Up Martial Arts I agree by signing this to assume all risk of personal injury that my child (I, myself) may suffer while participating in Level Up Martial Arts and all related activities (“Level Up Martial Arts”) and do hereby release and discharge Level Up Martial Arts and all of its officers, directors, employees and volunteers individually and collectively from all liability, including claims, expenses, causes of action, damages and lawsuits at law, or in equity for any injury or damage which my child (I ,myself) may suffer as a result directly or indirectly from participation in Level Up Martial Arts. In the event my child (I, myself) is injured or ill, I hereby grant permission to Level Up Martial Arts and/or Level Up Martial Arts officials and employees to render, secure, and/or authorize necessary medical treatment for my child or (I, myself).I grant permission to Level Up Martial Arts for my child (I, myself) (a) to be photographed, recorded and/or interviewed in connection with the Level Up Martial Arts and for the photographs, recordings and interview materials to be used and released publicly and (b) to participate in an evaluation of Level Up Martial Arts conducted by an external evaluator. (All evaluation participants are guaranteed confidentiality; the evaluator will not share individual student information collected during the evaluation process with anyone and the information and will not be used as part of Level Up Martial Arts assessment process.) I agree to participate in Level Up Martial Arts and the assessment process without financial remuneration, and I understand that this releases Level Up Martial Arts and any photographer/interviewer/evaluator from any future claims, as well as from any liability arising from the use of said photographs, recordings, interviews and evaluations. I also grant permission for Level Up Martial Arts to contact my child (I, myself) in the future concerning Level Up Martial Arts and related events and opportunities.
Payment / Program Information Agreement
Payments: Monthly / Full Auto Draft Payments will be drawn out through Square for Contract agreements on the 1st day each month. Contract included on first invoice. Checks: Make checks payable to Level Up Initiatives LLC with students name & school in memo. Returned draft fee’s / checks results in $35 Fee. Notification: Please notify by email or in writing if your child will be withdrawing 30 days prior to leaving the program. Monthly Contracts that are terminated before agreed upon contract date will result in a $65 cancelation fee. Invoices will continue otherwise for Contract / Auto Draft options only. Late Fee's: Outstanding balances will result in no classes for student until fully paid. Overdue Invoices later than 7 days receive $25 fee. Program Info: Tuition is same every month. No refunds on uniforms once received. Even if student withdrawals from program. Spring Belt Ceremony Fee’s will be billed $75 in February. Spring Belt Ceremony Fee, Fall Testing Fee, & Stick Combat Tournament Fee is Mandatory for all students. Instructor Notifications: Emails will be sent if your Level Up Instructor is absent from teaching for the day with info and class make up time & dates. Holidays/Closing are followed by your local ISD district. Please read and type if you agree/understand for the above & below policies.
Vendor Policy
As an approved vendor by the school, our program and instructors have signed all necessary documentation / policies as requested from the school to work with their students within their school during school hours & school acitvities.
Our program is unique from the others. We teach our students skills & techniques that can cause serious harm to others. There's an expectation we set for our students when it comes to the use of Martial Arts and hitting. We do our absolute best to keep those expecatations realisitc as possible with their age in mind so that our Little Warriors are set up for success. Without those expectations set in place, our Martial Arts program would not exist in a daycare / private school setting.
Zero Tolerance Hitting Policy
Level Up Martial Arts Little Warriors Program strives for safety. We have our Zero Tolerance Policy in place to protect our students, our Little Warrior families, other students not enrolled in the program, our schools and their staff, our instructors, and our program as a whole.
Students are reminded each class not to use their Martial Arts / Karate on their friends, families, or teachers. We drill this over and over to ensure that students understand that Martial Arts / Karate can cause serious damage to others if used incorrectly and inappropriately.
We treat each incident involving hitting on a case-by-case basis. We completely understand that we work with young children ages 3-5 years old and that each situation / incident is different. Our communication with teachers, directors, staff, and parents is vital to make sure all parties involved are safe and handled with swift discernment.
Zero Tolerance Policy
- Any Little Warrior student that hits or strikes another student or teacher in class intentionally or through horseplay and is reported by a schoolteacher, a director, or franchise owner, your child will not be able to attend class that day. We will gather as much information and facts as possible from the individual reporting the hitting & relay it directly back to the parents after class.
- Communication with parents will take place as soon as we get into our car in the parking lot after class has ended. We will call the parent involved and inform them by either leaving a voicemail, speaking directly through phone, or email. We will inform the front office of the incident that has taken place and make them aware of any actions taken on our end.
- If a Little Warrior continues to use their Martial Arts / Karate on friends, teachers, parents, or family members and is reported to the instructor by a teacher, director, staff member, or parent, we will suspend the student for one month from our Little Warrior Program. Parents will not be charged for this month.
- Upon arrival from the one-month suspension and the hitting continues, we will have no choice but to suspend the student from our program indefinitely.
Martial Arts / Karate is all about learning and practicing discipline, respect for others, and protecting our selves, our friends, and our family. We strive to make sure the material and skills our Little Warriors learn is not intended to cause any damage to those around us.

The above liability waiver applies to all families enrolling in the LUMA Sportz Program.
